About Us
The Healthy Communities Initiative (HCI)
The HCI team is dedicated to delivering direct, hands-on solutions to underserved rural communities. Our model deploys trained Community Health Workers (CHWs) into targeted neighborhoods to engage residents door-to-door, identify unmet medical and social needs, and connect individuals with the services that can improve their health and quality of life.

We focus on action: identifying communities with persistent poor health outcomes, implementing short-term, high-impact interventions, and supporting local organizations in sustaining these efforts long after our team steps back.

Our leadership and field teams bring deep experience implementing this model in high-need areas—including multiple Census Blocks in Phillips County, Arkansas—proving that community-based engagement can dramatically improve health awareness, service utilization, and trust in underserved populations.

Whether we’re launching a new site, training CHWs, or advising partners on best practices, our goal is the same: deliver practical, place-based solutions that close health gaps—fast.

Our Mission
To improve health outcomes by deploying community-driven strategies that directly address barriers to care, eliminate disparities, and elevate health equity across populations. We do this by supporting the people and organizations closest to the challenge—with knowledge, tools, and partnership.

Our Focus Areas

Empowering Community-Based Organizations (CBOs): We offer technical assistance, training, and partnership opportunities to strengthen the capacity of grassroots and regional CBOs. From program design to implementation, we ensure local organizations are equipped to lead the charge for change.

Deploying and Sustaining Community Health Workers (CHWs): HCI supports CHWs who serve as trusted liaisons between the healthcare system and the community. These frontline workers build relationships, provide health education, and connect individuals to critical services.

Facilitating Care Coordination through Case Management: Our structured case management workflow brings together CHWs, healthcare providers, and service agencies to coordinate care, address gaps, and ensure that each client receives wraparound, person-centered support.

Grounding Decisions in Community Voice via Community Action Boards (CABs) are the cornerstone of our model. These resident-led advisory bodies ensure that program priorities, outreach strategies, and evaluation efforts are guided by the lived experiences of the people most affected.
